Biometric Screening
Biometric Screening
All employees are eligible to participate in this personal and CONFIDENTIAL wellbeing assessment. Employees on the ACSD health plan who complete the assessment (biometric screening and Health & Lifestyle Survey) will be eligible to earn a $100 premium discount.
Information is emailed to all employees annually with dates and times to participate in the screening.
The Biometric Screening Wellness Program is completely voluntary and your privacy is of the utmost importance. All personal health information is kept confidential by CHC who follows federally regulated HIPAA guidelines. This ensures your PHI is not shared with your employer. The test will screen these areas:
Kidney Disease
Liver & Gallbladder
Heart Disease & Stroke
Anemia, Infections & Certain Cancers
Diabetes
Nutritional & Gastronintestinal Disorders
A Health & Lifestyle Survey is a necessary piece of the screening and the member portal includes personalized resources in the areas of activity, nutrition, hydration, sleep, a habit builder app, mindfulness resources, a work well series and financial wellness information.

Our electronic referral platform efficiently refers patients with opioid use disorder from emergency departments, OB/GYN offices, correctional facilities, inpatient units, pre-hospital settings, etc. to community based clinics. Our goal is to provide every person living with substance use and mental health disorders multiple pathways to treatment and resources within their own community by creating an extensive network focused on improving access and eliminating financial, geographical and social barriers.
