School-based therapy services are available for Tomorrow River School District students at school in the Hope Office. School-based therapy is provided by a licensed therapist and will be billed to your insurance. If the cost of therapy is a hardship or you have any questions, please contact your child’s school counselor.
If you are interested in starting school-based therapy services for your child, you may follow the steps below to sign up directly with our school-based provider:
Steps to Sign Up for the Hope for Tomorrow School-Based Therapy Program:
Click this link: Central Wisconsin Mental Health Associates and scroll down to click on the "request appointment" purple box.
Fill in the first & last name box with the name of the student who will be receiving services. In the same text box, please include the name of the parent/guardian in parentheses.
Example: Freddie Falcon (Parent/Guardian: Sherry Falcon)
Fill in the student’s Date of Birth
Fill in parent’s/guardian's phone number and email address
Check the appropriate box below Subject: Request Appointment
In the share who referred you to CWMHA section, please list the name, phone number and email address of your school counselor/school psychologist (see below).
*Including school's contact information starts the collaboration process.

Please note this is an initial contact, not setting up an appointment. CWMHA will contact parent/guardian to discuss the next steps, including additional paperwork. Please keep in mind, there may be a waiting list before your child will be assigned a therapist.
