Children who are diagnosed with asthma are required to have an Asthma Action Plan completed by the treating physician each school year. The school nurse is committed to working closely with parents and children on the proper management of this condition to ensure the child's safety and well-being while at school. If the child is able to self-administer medication, the Asthma-Anaphylaxis Self-Administration Form must be completed as well. See Printable Medical Forms page here.
