Indicator 13: This indicator tracks the proportion of 16+ year old youth with IEPs that have comprehensive, updated, and age-appropriate transition plans. The plans should include measurable postsecondary goals, relevant courses of study, and annual IEP goals related to transition services. The goal of Indicator 13 is to ensure that all youth with IEPs have well-rounded transition plans that support their path to postsecondary success.
Evidence-based practices in secondary transition refer to a set of strategies and interventions that have been proven effective through rigorous research in helping students with disabilities successfully transition from high school to post-school life. These practices focus on areas such as career exploration and planning, self-advocacy and self-determination, community involvement and support, and individualized instruction and support. By using evidence-based practices, educators, families, and other individuals can help ensure that students with disabilities have the necessary skills and resources to achieve their post-secondary goals and lead fulfilling lives as independent adults.
