I am an in-network provider for Medicare only.

I am an out-of-network provider with all other insurance companies. If your insurance policy provides you with out-of-network benefits, you may submit a claim and potentially be reimbursed a portion of the cost. I can provide documentation for you to submit to your insurance company for out-of-network benefit coverage. Please contact your insurance company to verify your out-of-network benefits.
