In order to ensure that we have up to date information about your child, please could you complete and submit the Pupil Information Form before your child's first day with us.
If you have more than one child joining Albyn School Nursery, please complete a separate form for each child.

This form will take approximately 5 - 10 minutes to complete. Your answers are saved automatically if you are logged into Google, meaning you can leave and come back at a time that is convenient for you. If you are not logged into Google, your answers will not be saved for you to come back to.
Please note that in order to complete the form, you will need the following:
Your child's passport details
A passport style photograph of your child
A copy of your child's visa documentation if applicable
Emergency contact details (other than parental)
Child's GP and medical information
Child's Health Visitor details
