Health Information Form - 

Complete and update yearly if your child has a health condition that needs attention from the School Nurse and/or teacher(s)

Prescription Medication Form - 

Complete if you would like your child to be given daily Prescription Medication while at school.

Also complete if your child will have a Prescription Medication to be kept in the nurse's office to use "as needed" (such as epi-pen or inhaler)

Parent Letter regarding Treats Allowed in the Classrooms due to potential Allergies
