All family members are required to read and sign the AJFWA Assumption of Risk Release & Waiver of Indemnity Agreement. Once all Family members have signed scan or take a clear picture of the completed form this will need to be uploaded during the online process.

A Vulnerable Sector Check and a Police Background Check is required by all new members over the age of 18 and updated every 5 years. Please take this FORM into your local law enforcement precinct. Once the check is complete please bring in to your Club Registrar. If you require a form that you can fill out on your computer click this FORM.
