Vaccine Registration
Follow the directions below to create an appointment for the COVID-19 Vaccine during the AFUHSD POD event.

Create an Account, or Login
Click on the registration link in the email invite you received.
click the "New user/Create a new Account here" link
Or, if you have already created an account, or received a 1st dose, click the "Login" button.
Enter Personal Info
If you are creating a new account, enter your name, birth date, and phone number, click the "Sign up" button.
Enter PERSONAL email address.
Enter your email address. This is VERY important: it is how your records are identified in the system for the 2 doses. Use a personal email address (not work), and avoid Cox email addresses.
Click the "Send verification code" button.
Click "Continue"; Enter Verification Code
Enter the verification code you received (or your email if you have previously signed up)
Click the "Continue" button.
Schedule a Vaccination
Click the "Schedule a vaccination" link.
Choose "Self"
Choose "Self" from the drop down. Click the "Next" button.
Select "COVID-19"
Check the "COVID-19" box.
Click the "Next" button.
Answer Questions
Complete the supplemental questions.
Click the "Next" button
Enter Personal Information
Enter your personal information.
Click "Next"
Click the "Next" button.
Enter Insurance Info (If you have it).
If you have insurance, you can enter the information here.
If you don't have insurance information, you can still register for no cost.
Click "Next"
Click the "Next" button.
Search for SPECIFIC POD Date (IMPORTANT)
To find the AFUHSD POD event, you MUST search for the specific date. The initial dates are 1/29/2021 and 1/30/2021.
Choose the "Abrazo_AFUHS Footprint 1st Dose" event. (This event will only appear if you have searched for the dates above)
You can also register to receive a second dose of the Pfizer vaccine on this date if you are in the correct window after your first dose.
The second round dates are 2/19/2021 and 2/20/2021.
Choose the "Abrazo_AFUHS Footprint 2nd Dose" event
Select Slot
Click the "Select Slot" link.
Choose Slot Option
Choose a time option from the available lists.
You can click the arrow on the right-hand side of the red bar to expand and view all the option.
Answer Screening Questions
Click "Next"
