COMPREHENSIVE PLUS
Affinity comprehensive PLUS is design for people who cannot afford extremely high monthly premiums, copays and deductibles but but need constant access to medical services.
Affinity comprehensive PLUS service includes:
Complete physical
Blood work
Base line EKG
Pap Smear or Prostate Exam
Lab Review
Rx Refill
Referrals
Real time access to your medical record

ADDITIONAL SERVICES**
ABI/PAD TEST
An ankle-brachial index (ABI) test is a simple way for our doctor to check how well your blood is flowing in your legs.
We use this test to check for peripheral artery disease (PAD)
Allergy Skin Test
Our allergy skin test, provide results of exactly 74 different types of allergens a human can suffer from. Our test can show which substances (allergens) are causing your allergic reaction.
Patient pay for the initial visit and ONLY a small amount every month for 12 months until next annual physical is due.
New cycle begins at the date of the original annual physical and same rules will apply if the patient wish to continue.
If patient miss three consecutive payments, plan is voided and will be consider as AFFINITY COMPREHENSIVE. Premium collected is non-refundable.
*This service is provided by our great partner SDM-1 STOP PRIMARY & URGENT CARE. The $ is due before the visit and is non-refundable once service is complete.
** These services are not included in our Affinity comprehensive package
*** Consultation, referral, Rx refill ONLY - Requested blood work and/or any other tests will be paid extra by the patient.
Note: We do not pay or reimburse for prescriptions
Affinity Health is not an insurance company. Appointment made through our partners( SDM-1) may be adjusted and may incur a "NO SHOW" fee. When appointment confirmed, please give the practice a 24h notice if you will not make it to scheduled visit.
