Access the Online Registration Form for SY 2025-2026.
Make sure that necessary information is provided in the form. This also serves as your child's enrollment form.
Prepare and submit the following requirements:
a. For Incoming Junior Kinder/Senior Kinder
Photocopy of PSA Birth Certificate
Previous Report Card (if any)
2x2 2pcs recent ID picture of the student (White Background)
b. For Incoming Grades 1 – 10
Photocopy of PSA Birth Certificate
ECCD Checklist for Incoming Grade One
Report Card (SF 9/Form 138) with LRN
Permanent Record (SF 10/Form 137) if available
Good Moral Certificate
2x2 2pcs recent ID picture of the student (White Background)
c. For Incoming Grade 7
In order to save a slot for the ESC (Government Subsidy) application, the following documents must be prepared and submitted to the school personally:
ESC Application forms to be filled-up on-site
Any of the following (whichever is applicable):
Latest Annual ITR/Certificate of Employment (for employed parents)
Non-filling of ITR/Municipal Certificate of Unemployment (for unemployed parents)
Affidavit of Financial Support stating Monthly Support Provided/Affidavit of Unemployment (for unemployed but with other sources of income

d. For Incoming Grades 7 Taguig Learner's Certificate (TLC) Scholars - ONSITE Application only
Original Student Application Form
Certified True Copy of Form 137/SF10
PSA Birth Certificate (Photocopied)
Original Certificate from previous school (from the adviser/principal, certifying that the student is graduating)
Parent’s Barangay Residency Certificate
Parent’s COMELEC Certification
Student’s School ID (Photocopied, both sides, in one page)
Please take note that health protocols are still implemented. (e.g. wearing of facemask and, bringing of alcohol and pen)
Proceed to payment of Tuition Fee and Books at the cashier. You may also be required to fill-up other important school forms.
Payment can also be done through our payment channels by clicking HERE.
An Official Receipt will be issued and you are now officially enrolled for SY 2025-2026.
Wait for advisories on class schedule, class list, Google Student Account details, and further instructions to be sent on the email address you provided. You will also be asked to join the ADSI Viber Community and Classroom Viber to receive updates on school opening.
Old students with back accounts must first settle their remaining balance from the last school year. If none, they may proceed to step 2.
