step 1: click on the link above and go to Athlete Testing

step 2: Sign in with Username: western Password: braves1

step 3: Enter your student ID -sport you are trying out for

step 4: Enter student ID - sport you are trying out for, full name, and date of birth

step 5: click on baseline, then click on concussion vital signs, then click on unsupervised or supervised by parent and alone.

Read the directions very carefully so you don't have to take the test again!!!

When you finish just logout and results will be sent to Coach Slaughter.
