These important forms must be turned in as soon as possible. If you are not sure if we have these forms on file for your student, please contact the front office.
All students and staff MUST review & answer each question YES or NO each morning before coming to school.
You do not need to consider any chronic conditions you may have.
Temperatures will also be checked upon entering campus each day.
Have you been told to quarantine/isolate by a medical provider or the health department?
In the last 14 days, have you had face-to-face contact within 6-ft for 15 minutes or more with someone who has or is suspected of having COVID-19?
In the last 14 days, have you traveled to a location with a Centers’ for Disease Control and Prevention (CDC) or Kansas COVID-19 travel warning?
Are you experiencing a new cough, shortness of breath or difficulty breathing or any of the following symptoms?
Fever of 100 F or above
Chills
Repeated shaking with chills
Muscle pain
Headache
Sore throat
Loss of sense of taste or smell
5. Was your temperature 100 F or above this morning?
If you answer YES to any question
Stay home. Do not come to school.
Call the front office right away to inform the receptionist.
Email all of your teachers.
Continue to attend your classes online.
If you have reported symptoms, please answer your phone when called to facilitate contact tracing process.
Contact your healthcare provider for an evaluation.
Return to school when you are cleared by your healthcare provider
