The ACGME requires you as a fellow to complete various types of evaluations. You'll be asked to complete evaluations of the program, faculty, and yourself throughout the course of your:
Self Assessment - completed every 6 months
Fellow of Program - completed every 6 month
fellow of Faculty - completed every 3 months
All evaluations, except your self assessment are anonymous. Below are the links to the evaluations you'll be asked to complete.
Evaluations are done by the faculty on a regular basis to help show what is going good and what could be improved upon. The following services are evaluated:

Set expectations at the beginning of the week.
Observe behaviors and divide into positive feedback and areas for growth
Use the Four box model to provide this feedback
Positive | Continue this | Do more of this
Negative | Do this differently | Do less of this
Your "this" should be very specific:
Focus on demonstrated actions and behaviors rather than on the person
Focus on descriptions instead of judgment
3. Provide next steps as specific directions and tasks for growth, instead of generalities.
4. Identify the specific outcomes you would like to see as a result of the tasks
5. Tell the learner why this is important.
Good feedback is....(with examples of wording in italics)
…based on explicit, pre-rotation expectations: "You should see every patient before rounds, and page me at the end of the day"
…focused on what went well: "I noticed you had your notes done before you left clinic - great job!"
…behavior-based and specific: "When the primary team told us they hadn't been spoken to about the plan, I felt like we may have missed an opportunity for improved care. Can you say a bit about that?"
…timely: Take specific notes - DON'T wait until the end of the day (or rotation!)
