Asthma Action Plan Information - Illinois Public Act 099-0843

As of January 1, 2017, Illinois Public Act 099-0843 is requiring schools to request from parents of students with asthma, an Asthma Action Plan (AAP). An Asthma Action Plan (or management plan) is a written plan that you develop with your child’s doctor to help control and treat your child’s asthma.

At Williamsville School District we feel it is crucial to have this form on file to help provide your child with the best care possible and to perform the physician recommended treatment in the event of an asthmatic episode. The Asthma Action Plan will inform us of daily treatments the student may be receiving and how to handle worsening asthma, or attacks.

A yearly updated Asthma Action Plan should be provided to the school building your child is currently attending. Please provide the physician completed form as soon as possible.

Illinois Certificate of Religious Exemption to required Immunizations and/or Examinations

On August 3, 2015 Public Act 099-0249 was enacted that changes the process for parents or legal guardians seeking a religious exemption to any required immunizations or examinations for their children for school attendance.

The new law requires parents/legal guardians who object on religious grounds to complete a Certificate of Religious Exemption. This certificate must also be signed by the physician, advanced practice nurse, or physician assistant responsible for performing the student examination.

Beginning October 16, 2015 the use of the Certificate of Religious Exemption form is required for all students entering kindergarten, sixth or ninth grades when parents or legal guardians seek religious exemption and for all newly enrolling students requesting religious exemption on or after October 16, 2015.

Failure to comply with this requirement on or before October 15 of the school year will result in a student’s exclusion from school attendance until the completed form is provided to the school office.

Health care providers are not be expected to furnish this document. Please take a blank copy of the form with you to your scheduled appointment if seeking religious exemption.

Meningococcal Vaccination IDPH requirement for 12th grade and 6th grade

Beginning with the 2019-20 school year, the Illinois Department of Public Health requires those students entering into 12th grade to have two doses of meningococcal vaccine, unless the first dose was given after the age of 16 years. A letter was mailed to all incoming 12th grade students who are still missing the requirement. Incoming 6th grade students are required to have one dose. The vaccination needs to be done by the first day of the 2019-20 school year and medical verification given to the school. Within the Forms and Handouts section below you will find a parental letter regarding this matter from the IDPH.
