Participation Info

Complete physicals and concussion forms are required to be turned in to school before participation in any interscholastic sport (including track) is allowed.  If those forms have already been turned in for a different sport for the 2018-19 school year, there should be no problems.  Those forms can be found at the top of this link http://usd437.net/schools/wrms/sports.html.

We will go over the following paperwork the first day of practice, also posted below for your review:

1) Guidelines and Policies (see attachment below)

2) Permission Form (If agree with Guidelines and Policies, sign and return to head coach between Mon. March 4 and Thurs., March 7 see attachment below)
