Safety Forms

The Upper Grand District School Board has created two forms to help ensure student safety.  These forms can be found below and must be returned signed by a parent or guardian before students will be allowed to participate in any lab activity.  Thank you for getting these back as soon as possible.  In addition, I would like to be able to post some of our work that we do in class on the website.  In order to allow your student's work to be posted (using only first names) I would ask that you sign a release form that allows me to share the great things our class will be doing.   Paper copies of both of these forms have been sent home with your student, but in case they didn't make it I have posted them below as both a Google document and a pdf.  Thank you again.
