**Franklin Jr/Sr High School Athletic Registration (CIPPE) ** 

 Section 6 must be filled out by a medical provider and uploaded to your online registration.  Please call the athletic if help is needed

PIAA Section 5 & 6 

PIAA Section 8 Recertification by Licensed Physician of Medicine or Osteopathic Medicine (must be completed by a DO or MD following an injury)

PIAA Section 8 Recertification Form (Return to Play Form).
