**Franklin Jr/Sr High School Athletic Registration (CIPPE) **
If you choose to have your Doctor or Urgent Care do you physical, section 5 & 6 must be filled out and uploaded to your registration form.
PIAA Section 8 Recertification by Licensed Physician of Medicine or Osteopathic Medicine (must be completed by a DO or MD following an injury)
PIAA Section 8 Recertification Form (Return to Play Form).
