Asthma in-home assessment tool

The Colorado Healthy Housing Coalition has created a standard questionnaire for trained environmental health professionals conducting environmental home visits for children with asthma. The tools are fully customizable and intended to help identify possible sources of exposure and asthma exacerbation. Being invited into someone’s home to conduct a home health assessment is a rare privilege.  Please take the time to personalize this checklist to best meet your needs and individual style. 

The Community Preventive Services Task Force recommends the use of home-based multi-trigger, multicomponent interventions with an environmental focus for children and adolescents with asthma based on strong evidence of effectiveness in improving overall quality of life and productivity, specifically:

• Improving asthma symptoms

• Reducing the number of school days missed due to asthma
