All Athletes must have the following completed in order to participate in our UIL Athletic Programs.

*Completed Physical form on file at Bammel Middle School (Physical forms are available in the front office)

*All Rank One online forms signed and completed

*Appropriate attire for their specific sport as defined by the Head Coach of each sport.
