All Sports Physical and Health History Update forms must be submitted to the High School Nurse.
Physicals must be from within 1 year of the date of tryouts/first practice.
A new Health History Update form must be completed for each sports season (fall, winter, spring) and is valid for 90 days.
Physicals are due by
Fall Season: July 15
Winter Season: October 24
Spring Season: February 16
