Scrip Family Agreement

All families who order Scrip must have an agreement on file with us.

This tells us how to distribute your rebates.

Please download and fill out the attached form.

You can turn the form in at school or scan and email the completed form to us at scrip@sfhg.org. Forms turned in at school should be addressed to Laura McNicholas/Scrip Program.

If you have not yet signed up for the Scrip Program, please email us your contact information and request the enrollment code at scrip@sfhg.org

another unless you are changing your rebate distribution preferences.
