3. Application Materials: Unofficial transcripts and proof of degree. Scan and email to Dr. Cherissa Vitter.

4. Immunization form: If you do not have recent immunization records, you may sign the waiver. Just scroll down to the 2nd page for the signature line. Email the form to Dr. Cherissa Vitter.
