Please note:

• • Medication must be brought to school in the original pharmaceutical container.

  • Over the counter medication must be brought in its original unopened container.

  • We will not administer any medication sent to school in baggies, envelopes, etc.

  • The parent MUST report immediately any changes in prescription or dosage.  

  • New doctor's orders must be obtained for each change. 

  • Medication permission must be renewed at the beginning of each school year.

  • Cough drops are permissible if they do not contain any ACTIVE ingredients.  A parent or guardian must send in the cough drops in the original packaging with written permission for their student to have them. Cough drops that contain ACTIVE ingredients are treated like any other medication.  They require parent and doctor written permission.  This is an Illinois State Law.

Medication at the End of the Year:  ALL Medication must be picked up by a parent or guardian including inhalers unless the doctor and parent has specified that the student may self carry the medication.  

Any medication that is not picked up by the last day of school will be disposed of.

Self Carrying Medication: All medication is stored in the Health Office.  If your child will carry and self administer his/her inhaler, their physician must specify this on the medication permission form and as parent or guardian you initial the line giving permission for your child to self carry.

Please have your child's physician complete the corresponding care plan linked below if your child has one of the following diagnoses:

• Asthma

• Diabetes

• Food Allergy

• Insect Sting Allergy

• Migraine

• Seizure

• VP Shunt

Your child may be excused from PE class for 2 days with a parent note.  To be excused any longer requires a note from the doctor.  Please note: NO PE  = NO RECESS.

If your child's injury requires the use of crutches or any special instructions, such as extra time in between classes, please have your doctor specify this on his/her care instructions.

  Approved foods that may be brought to school for a snack in the classroom:

• • Fruit (fresh or dried including Raisins & Applesauce)

  • Vegetables

  • Pretzels/Plain Popcorn

  • Goldfish/Graham Crackers/Saltine Crackers/Animal Crackers

  • Plain Cereal

In accordance with the D44 Wellness and Food Allergy policy, only the foods shown above may be brought to school for a snack in the classroom.  There are many students in the building with severe food allergies and because of this, snacks cannot be shared with other students.  Snacks that are made in a plant where peanuts and or nuts are processed are not allowed in the classroom.
