The District 44 Health Office Staff is committed to taking the best care possible of your child.  Please be sure that we have a phone number to reach someone that is available to pick up your child in case of illness or injury.   Please keep us aware of any health concerns that your child has.

Immunizations

All immunizations must be current according to the requirement of the Illinois School Code.  The immunization record is part of the physical but if your child's is separate, you can email or fax it to the school nurse.

Dental Examination

Students in kindergarten, second, and sixth grade must present proof of having been examined by a licensed dentist or a waiver within 18 months of May 15th of that school year. 

Sports Physical

Students must have a current sports physical to participate in interscholastic sports.  The physical must have been performed within one year of the start of the specific sport and must be turned in before tryouts. 

Medication

A school medication permission form must be completed by both physician and parent/guardian, and is necessary for prescribed and over the counter medication, including cough drops, that is to be given to a child during school hours. Some medications may also require a care plan. Care plan forms are available in the next section in both English and Spanish.

Please note:

Medication at the End of the Year:  ALL Medication must be picked up by a parent or guardian including inhalers unless the doctor and parent has specified that the student may self carry the medication.  

Any medication that is not picked up by the last day of school will be disposed of.

Self Carrying Medication: All medication is stored in the Health Office.  If your child will carry and self administer his/her inhaler, their physician must specify this on the medication permission form and as parent or guardian you initial the line giving permission for your child to self carry.

Physical Education Excuse

Your child may be excused from PE class for 2 days with a parent note.  To be excused any longer requires a note from the doctor.  Please note: NO PE  = NO RECESS.

Dealing with Injuries at School

If your child's injury requires the use of crutches or any special instructions, such as extra time in between classes, please have your doctor specify this on his/her care instructions.

Approved Snacks

  Approved foods that may be brought to school for a snack in the classroom:

In accordance with the D44 Wellness and Food Allergy policy, only the foods shown above may be brought to school for a snack in the classroom.  There are many students in the building with severe food allergies and because of this, snacks cannot be shared with other students.  Snacks that are made in a plant where peanuts and or nuts are processed are not allowed in the classroom.
