Welcome back to school students and families. I hope you enjoyed the beautiful summer we had and are looking forward to an amazing fall.
MVS will not be offering a flu vaccine clinic this year. I was informed by the state that the nasal mist is not available. I am sure most students will want a parent present during an injection and that will be much easier to do at your healthcare provider's office.
The nurse's office will be open from 8 am to 2:45 pm Monday-Friday.
All 4th grade and new students need to have a physical on file. If you received a note at the end of last school year from me please follow up with your healthcare provider. I will send home a 2nd reminder soon.
If your child takes a prescription medication or needs an epi-pen please get prescriptions and emergency plans from your health care provider. I am not able to administer any prescription medication without the appropriate documentation. Parents and physicians need to sign emergency plans.
All 11 year olds need a Tdap booster. Please have a copy sent to school when this is done.
Please fill out and send back the over the counter medication form that will go home on the 1st day of school. Having this on file really helps with time management and not having to track down parents while they are at work.
Thank you for helping me keep the students happy, healthy and safe.
