By The end of class on Tuesday May 01, 2012, you must submit the following 2 AWARD APPLICATION forms printed out and signed by your parents:

1. Greenhand or Chapter Degree Application

2. A Proficiency Award Application for: LINK: PROFICIENCY APPLICATION FORMS CLICK HERE

   1. Placement, or Entreprenuership or Reseach

   2. Resume for the award

   3. Proof of Project up to 6 pictures with you in the photo perfroming 6 different skills related to your award application. (Up to 50 word captions for each)

   4. Attach a FULL AET RECORD BOOK REPORT to the application

   5. You may NOT apply for a local proficiency award you have won in a previous year.

RESULTS WILL BE ANNOUNCED AT BANQUET MAY 09TH WEDNESDAY
