All reimbursements should be requested NO MORE than 30 days from the date of purchase

For mini grants requests, please us this Google Form 

Please use one of the following forms when processing your reimbursement through Harvest PTA.  You will use these forms if turning in a PTA-related expense either from a volunteer, committee coordinator or staff member.  

Please remember to use your tax exempt form when shopping to tax advantage of our Harvest's PTA tax exemption shopping.  In MOST cases, tax will not be reimbursed.
