To opt out, please send a letter stating you would like to opt out of the screenings and then I would need a doctor's form stating that they were screened this year for vision and hearing.

WE DO NOT PERFORM POSTURAL SCREENINGS.

Medication reminders:

All medication orders  must be renewed by a physician ANNUALLY

All Epipens and allergy plans are required on the first day of school.

REMINDER

All missing information requested is due no later than 9/30. Some of the requested information, if not turned in by due date, will result in your child being excluded from class until received.
