KHS/MSK RANK ONE INFORMATION FOR ATHLETICS

Please see below the information for completing the RANK ONE online forms for the upcoming school year. There is also information listed regarding physical information.   All online forms and physical information must be completed before the start of the Fall sports season.

On-line Information and Physical Information for Athletics

All athletic forms will now be completed online and need to be completed by August 19th for KHS fall sports. 

 The instructions are below:

All Kennebunk High School athletes (9th, 10th 11th and 12th graders) and Middle School athletes (6th, 7th, and 8th graders) will need to complete the forms below.

You will need the following information before you start:

1. Your student’s 5 digit Power School ID

2. Date/Month of the student’s last Tetanus (TD) booster

3. Your Insurance Company, Policy Number and Name on Policy

Forms can be accessed by going to https://maineregional.rankonesport.com/New/NewInstructionsPage.aspx

1. First:  go to Download and Print.

   1. All incoming 9th graders will need an updated physical. The KHS Medical Release Form is all you need to get signed when you get your child’s physical. If your athlete has had a physical after January 1, 2023, please have your physician sign off on this form or have a signed note from the physician’s office stating that the student is cleared to participate in athletics.

   2. Physicals are valid for 2 years from the date of the exam. If your child’s physical is going to expire prior to or during the 2024-2025 school year, please send an updated release after the physical.

   3. Read the KHS Athletic Handbook and the Heads Up on Concussions information.

2. Second:  go to Electronic Participation Form

1. Fill in all areas on the Athletic Card.

2. Sign off indicating that you and your student have read the Athletic Handbook and all of the Concussions information.

3. Go directly to the Concussion Management page. Please read all of the information and then sign off at the bottom.

1. You have now completed all online forms and will receive an email from Rank One.

2. You will need to deliver their physical form to the athletic office or fax it to 207-985-1355.
