Documents for Participation

Marquette uses Final Forms, a web-based service that allows digital access to emergency info, as well as simplifies paperwork from year to year, sport to sport.  All physicals for the 2025-26 season must have been completed after June 1st of 2025 

Please use the following link to our Athletics website for all the paperwork you'll need to participate.  You MUST be cleared before being allowed to practice!
