NOTE: No cash or checks will be accepted. Dues are exclusively paid online. The $25 does NOT include t-shirt payment.

Step 3: Parent Permission Form

Have your parent fill out the parent permission form with their information! Even if you have filled this out for another club, make sure to fill it out for Hosa as well!
