Important message regarding medications:
Every student going on the trip should have this form completed prior to our departure on May 27, 2025.
- In some cases, children take both prescribed and over-the-counter medications. Please make sure that all forms for each medication are filled out in completion.
Completed forms should be hand delivered to me, Carrington, as soon as possible. This will give me an opportunity to review forms and ensure that all information is accurate.
All medicines must be in their original containers and hand-delivered to Ms. Light before we get on the bus on May 27, 2025 (do not send pill boxes or a Ziploc bag of goodies). Please only send enough to be used for the duration of our trip: May 27-29, 2025.
I know all of this can be confusing. Please let me know if you have any questions.
