ATHLETIC MEDICAL REQUIREMENTS

Before each athletic season (Fall, Winter, Spring), athletes must fulfill two requirements described below:

1) PHYSICALS:  All student-athletes must have documentation of a current physical on file with the school nurse.  Physicals are good for 13 months from the date of the exam.  Students with expired physicals will not be permitted to participate with their team until a new exam is complete and on file with the school nurse.  Please contact the school nurse to assure that your child’s physical is up to date and that current records are on file with the school nurse.

2) FAMILY ID:  Parents/Guardians are responsible for completing the web-based registration process in which they will be asked to ensure that any and all relevant medical information concerning their child’s health, medical concerns, injuries, medication and allergies are reported. As part of the registration process this year we are providing parents and student-athletes with basic information regarding concussions and head injuries.  As part of the registration process you will be asked to read the information provided and attest to the fact that you have read the information.
