DOCTOR'S PHYSICAL - Players are required to have a doctor's physical before being able to play at all, even at practice. This is a liability issue and we CANNOT let players participate without having that completed.
UNIFORMS 2024 - Players need to order a t-shirt to wear for games. The cost is $18 for the shirt and numbers will be assigned at random. If you have trouble paying- contact coaches, we will help.

ADDITIONAL VOLLEYBALL GEAR - Players may wish to have knee pads, which you can find at most sporting goods stores. Knee pads are not required, but are helpful in avoiding nasty floor burns (ouch!). For their safety, players must have tennis shoes - they will not be allowed to play barefoot, in socks, or in sandals.
Badger Sporting Goods- OPTIONAL ORDER--2025-- There are other OMS Volleyball t-shirts, sweatshirts, sweatpants, etc. on a separate order form, which will be filled out ONLINE. These items are optional, and there is no paperwork to turn in to OMS or the coaches (you complete the order online).

DEADLINE- ASAP! (these optional orders take a few weeks to come in. Do not expect them right away for the season)
Parent Permission to travel Form- this needs to be filled out and turned in before our first game of the season. Please either email this form back to the coaches or print a copy and have your player bring it to practice to turn in.
You only need to fill out this form once for all of the games (one form covers the whole season). This gives us a contact phone number at away games. It also informs us which players have permission to travel.
If you are ok with other families driving for your player, please mark at the bottom of the form that you "approve of all drivers". This allows your player to ride home with other families to help with carpooling.
