Will the program coordinate & process our licensing paperwork?
No. That is your responsibility. But we will assist with the education verification component, if there is any. If any part of your state's paperwork involves a signature or seal from the program or university, upload that part of the paperwork using the upload link provided further down on this page. Do not email anything.
Do I have to pass the boards first before starting on my licensing paperwork?
No. For most states, they will open a file for you when they receive any paperwork. Once you've passed the boards, the paperwork already on file will be finalized and your license will be issued.
When do I submit the paperwork to you?
Entirely up to you and your research of your state's requirements. But remember to take into account several weeks of processing time by the registrar and by the State Dept. Requests to "rush" any documents cannot be entertained by the registrar. Please note that if the form asks for confirmation of graduation, the registrar will hold on to it until you've actually graduated.
What's the licensing application procedure for my state(s)?
You will have to investigate via each state board's website.

I have questions about how to complete the paperwork. Can you provide guidance?
Unfortunately, no. ALL questions should be directed to the state board.
However, the following information might be helpful when filling in the fields:

I will have rotations going on past May. Can I send you my licensing paperwork now?
SUBMIT THE FORMS BEFORE MAY 1. This will allow time for the registrar to process your forms by your graduation date.
But please note that except for New York State, most states require an attestation that you have graduated. If you still have grades pending after May, you are likely an August 31 or a December 31 graduate. The registrar will not send out your form until they are able to attest that you are officially graduated.
EXCEPTION: New York allows preliminary confirmation that you've completed all requirements to graduate. See the New York section of this page below.
How do I send you the paperwork?
FIRST, look at the paperwork required by your state. Does the form REALLY require a signature/seal from New York Tech?
If not, don't send it to us.
Do the following ONLY if a form requires a signature/seal from the university:
Separate the relevant pages from the rest. Forms that do not require a signature or seal from the university will be discarded without notification.
Usually, there is section on the form that you have to complete first, before sending it to us (including any signatures, dates, and photos) . If asked, your graduation date is May 31, 2025and your degree is MS Physician Assistant Studies.
Scan/save as a PDF. Name it as follows "Last Name, First Name, Licensing State" (e.g. Smith, Lisa California)
If you don't get a confirmation email within 30 minutes, it didn't go through. Check your spam folder.
IMPORTANT STEP: FORWARD the confirmation email to ctesorie@nyit.edu with the message "Please process my licensing form, which has been uploaded to the Drive". This will alert the program manager that there is a form waiting to be process. Skipping this step may delay processing significantly!
Can I mail the documents to you?
No, we prefer the electronic procedure. Even if your state does not accept electronic submissions, the unprocessed form can still be sent electronically to the program.
How do I ensure that a transcript is attached to my paperwork?
A transcript will NOT be attached to your form. Even if the licensing form says "attach transcript", it cannot be done. Instead, you have to go through the separate process of ordering a transcript through the registrar, for direct delivery to the state department.
The above method of ordering the transcript separately has not caused any issues with the licensing boards in the past. Before ordering, it is your responsibility to check whether the state accepts eTranscripts or if they prefer the traditional mailed version.
If ordering prior to graduation, select "Hold for Degree" or "After Degree is Awarded" in the transcript request form, and the registrar will wait until your graduation is listed before sending the transcript. More on ordering transcripts >>
The registrar will ALWAYS email you once your form has been sent out. We are not copied on that email. They will also indicate on your myNYIT Student HUB that it has been processed.
What happens next?
If there are any issues, the registrar will email you. Typical issues include discrepancies in how your name is listed on the system and an unresolved Bursar bill (could be anything from unpaid tuition, a parking ticket or a library fine!).
After May 31st, the registrar will affix the official college seal on the forms and email/mail them to the relevant states.
After that date, (maybe 2-3 weeks later) you can start checking on your myNYIT Student HUB portal to find out whether the forms have been mailed. (see diagram below). There will a tracking number included (if mailed). If emailed, that section will indicate the email address it was sent to and the date.
If your license is "late", check with the state first. Look on their website to make sure that you've completed your part of the process (such as sending in transcripts, fees or any other required forms). Often, the form that requires college completion is not the only form they require. If they say that they never received the form that requires completion by the college, give them the above-mentioned delivery details from your Student HUB page. More often than not, they will magically find the form. Contact me if it needs to be re-delivered.
NEW YORK STATE
Read all the above instructions before reading this NY section.
Know your documents. You are responsible for reading the full instructions for New York State here: http://www.op.nysed.gov/prof/med/rpa.htm. Multiple documents and a fee are required by the state. The program will only process Form 2. You are responsible for submitting the other documents/fees directly to the state.
All questions about how to complete the forms should be directed to the NY state department. We don't have the expertise to answer those questions or the resources to keep up-to-date on any procedural changes. Similarly, we do not have the ability to intervene on your behalf for any issues.
Form 2 ("CERTIFICATION OF PROFESSIONAL EDUCATION"): This is the only form that requires information from the PA program and a seal from the registrar. Again, do not send me any other NY State forms.
Complete every section on Page 1. Leave Page 2 blank. Incomplete forms cannot be processed.
Section 3, Page 1: Write the name you want to be on your license. Check with the state regarding what is permitted, if different than the name on your state-issued ID (or passport). This will end up on your license.
Section 5, Page 1: This is the name on your diploma/transcript. This name will NOT end up on your license. The only purpose for this question is so NY state can match your transcript to your file when they receive it. Check it first before completing. I don't have access to check it for you. If you fill this part incorrectly, the registrar will hold up your licensing form. Order your own transcript. NY state requires that a transcript be attached to Form 2 but the registrar will NOT be attaching a transcript. Instead, you have to order it separately, for direct delivery to the state department. More on this >>
Be sure to check "Hold for Degree" to ensure that the notation "Degree Awarded" is included on the transcript when it reaches the state board.
IMPORTANT NOTE FOR AUGUST OR DECEMBER GRADUATES:
If your rotations run past May, inform me AND submit two Form 2s
The first one will be sent upstate to confirm that you've completed all requirements to graduate. For this to happen, you MUST inform me once all your grades are posted.
The second one will sent as soon as you've officially graduated (either August 31 or December 31, based on how late your rotations end)
