להלן הטפסים שיש למלא:

• טופס ויתור סודיות
• טופס הצהרת בריאות
• טופס אישור פרסום תמונות
• שאלון היכרות
• טופס הנחיות בריאות לתלמידים במצבי בריאות מיוחדים - ימולא ע"י רופא רק עבור תלמיד/ה במצב בריאות מיוחד

את הטפסים המלאים ניתן להגיש בשני אופנים:

• בדוא"ל לכתובת d.noam15@gmail.com בנושא המייל כתבו "רישום לכיתה א".
• להכניסם למעטפה סגורה, לכתוב על גביה "רישום לכיתה א" ולהגישה למזכירות בית הספר.

Dear parents,

Following is the list of forms that needs to be completed:

• Medical confidentiality waiver
• Health declaration
• Photo release form
• Introduction questioner

* Doctors instructions for children with special healthcare needs

The completed forms may be submitted by either one of these options:

* As an E-Mail attachment to d.noam15@gmail.com. Please include the words "רישום לכיתה א" in your message's subject field.

* Enclose them in an envelope, annotate it "רישום לכיתה א", and submit it to the school's office.
