The Athletic Department at Myrtle Grove Middle School now hosts their Pre-Participation Athletic Forms online with PlanetHS. This digital platform will allow you, and your student(s), to complete and access athletic forms via computer, tablet, or mobile phone. It is HIPAA, COPPA and FERPA compliant. Below is a kick off letter that will give you step by step directions to to get your athlete enrolled. Also, attached is a YouTube video that will give additional support when singing up and uploading information.
