At the start of every school year the MVHS Health Office requires updated health forms for all students. If you student requires medications at school, including EpiPen's or inhalers, we also require a new prescription/MD. The appropriate forms can be found under the "FORMS" tab.

The State of New Hampshire Immunization Law requires that “children have proof of all required immunizations, or will have valid exemptions provided to the school, in order for the child to attend school”. We pride ourselves in keeping MVHS well-protected from dangerous communicable diseases as well as keeping students healthy. Below is the New Hampshire School Immunization Requirements for the 2025 / 2026 school year and religious exemption form.
