The Health Office requires an updated health forms on all students at the start of every school year. We also require a new prescription/MD order for a student with any diagnosis that requires nursing care or administration of a medication, that includes self carry epi-pens and inhalers. The appropriate forms can be found under the "FORMS" tab.

The State of New Hampshire Immunization Law requires that “children have proof of all required immunizations, or will have valid and notarized exemptions provided to the school, in order for the child to attend school”. Merrimack Valley High School achieved an immunization rate for the academic year 2019/2020 that was above state average.
