SAU #46 provides each employee with medical coverage through SAU #46 a 1095-B or 1095-C form stating that SAU #46 provided minimum essential coverage for the plan year. This IRS form is not required to be filed with individual taxes. As of January 1, 2025 SAU #46 is only required to provide 1095-B or 1095-C forms if requested by the employee.

If you would like to request a 1095-B or 1095-C form, please email Kathleen Boucher (kboucher@mvsdpride.org) and a form will be provided within 30 days.
