Primary School Medication Form

Students are not permitted to carry any prescribed or over the counter medications. All medication MUST be brought to the Health office by a parent/guardian in the original container

The parent /guardian is required to complete all paperwork before the nurse can administer medication in school (please print form from below)

 The parent/guardian is responsible for picking up any unused medication at the end of the school year
