This form must be submitted with a non-refundable $10 processing fee (check or money order payable to Millstone Township School District). Send to Gina Whalen, MPAC Managing Director, 18 Schoolhouse Road, Millstone Township, NJ 08510.

Applicants will receive notification of the availability of the date(s) requested within fifteen (15) working days of the receipt of the application. If the Performing Arts Center is available for the dates requested, you will receive a contract with specific requirements and the fee structure for your rental.
