Sports physicals are good for 365 days to the date of the exam and need to be current by the start of the first practice.

Please make sure the physical form is filled out in it's entirety including:

• Page 1- including a parent/guardian and student signature at the bottom

• Page 2- if not applicable, select "no" and sign the bottom

• Page 3 and 4- are for the physician to complete

Please drop off all original copies to the nurse's office.

**All physicals must be turned in prior to the first day of any sports practice, we recommend two weeks prior if possible to avoid any delay**
