Due July 15th!

Athletic Requirements Checklist (Due July 15th- online forms are not open yet for the new school year):

All Athletes Need to Turn in Required Online Athletic Forms by July 15th. You can complete these athletic forms here: https://lovejoyisd.rankonesport.com/New/Home.aspx (you do not need to print these forms)

All Athletes need to turn in the new LISD Medical History and Sports Physical form by July 15th.

LISD 2021 Sports Physical Information:

All cheerleaders must pick up the new LISD 2021 Sports Physical form from the WSMS front office to be completed by their doctor prior to attending summer camp on August 2nd-5th. Please turn this completed form into the WSMS front office by July 15th so it can be processed by the district's athletic department and recorded in PowerSchool.

Before turning in the form, please make a personal copy or take a picture of both sides for your records in case the form gets lost for some reason at the district level.
