How to Request FMLA at Legacy
Contact Human Resource for required documents.
Submit completed documents with required attachments.
Upon approval, submit Time Off in Skyward/Frontline (as applicable).
Update personal email address (2nd email) in Skyward.
This is where you will receive correspondence while on FMLA
Notify Human Resource if FMLA start date or return to work date changes.
Provide 'return to work' certification to HR upon return.
Throughout the FMLA process, the employee will receive various emails with detailed instructions from: hr@legacypca.com, payroll@legacypca.com, and/or benefits@legacypca.com. It is imperative to read all information provided to grasp a total understanding of both district & employee requirements.
