How to Request FMLA at Legacy
Contact Human Resource for required documents.
Submit completed documents with required attachments & await approval/denial letter.
Submit Time Off in Skyward/Frontline (as applicable).
Update personal email address (2nd email) in Skyward.
Notify HR if FMLA start date or return to work date changes.
Provide 'return to work' certification to HR upon return.
Throughout the FMLA process, the employee will receive various emails with detailed instructions from:
