This assessment page is to evaluate the teacher's portfolio as well as his/her CAD achievement. This form needs to be accessed by the head of the committee/board account to start evaluation. The head of committee/board must meet the other members and reach to an agreement about the score of each question. The given score is based on the intended teacher's achievements during this academic year that showed throughout the content of his/her portfolio website.
