Kindergarten Registration Requirements

First time in a Hawaii Public School:

Parent/Guardian Enrollment Checklist

• Requirement by First Day of School

  • Original Birth Certificate (Copy will be made by school staff member)

  • Parent Photo ID

  • Original Proof of Residency in our district: Current utility bill; dated within 60 days, with your name and physical address or Rental Agreement/Lease. (If living with a family, a copy of family member’s utility bill showing address and a notarized letter stating that child is living in their household.)

  • Tuberculosis (TB) clearance certificate or letter from Department of Health is required for entry into Hawaii schools.

    • Available for free at the Department of Health, 3040 Umi Street, Lihue HI 96766- call 241-3387 for an appointment 

  • Immunization record: Completed on Form 14 

  • Physical within one (1) year  or  appointment card from a physician’s office showing date and time of physical appointment.
