Title VI - Federal Indian Education Programs require the 506 (Indian Student Eligibility Certification) form to provide the official record of American Indian/Alaska Native students to be counted for the purposes of Title VI funding and services. 

If your student identifies as American Indian, please fill out the below 506 form and send it back to your AIE program classroom.
