Enrollment Forms

Below are enrollment forms for children that are 3 or 4 years of age as of September 1st of current school year.

*Children who are 4 years of age are eligible to enroll in Voluntary Pre-Kindergarten classes 5 half days a week (morning/afternoon)

Monday-Friday, from 7:50am-11:00am or Monday-Friday from 12:00pm-3:10pm

*Children who are 3 years of age are eligible to enroll in School Readiness classes 2 half days a week

Mon./Wed.. from 8:00am-11:00am or Tues./Thurs.. from 8:00am-11:00am
