Step #2= Fill in top part with name and what type of shirt you want, size, and check the box that says they are authorized to use all logos listed (You cannot change any logos or add any to the jersey for this special pricing) ***See the pricing on the right side of website.  IGNORE the other boxes on the order form.

Step #3= Pay with credit card (all shirts will be shipped to Coach Reed to avoid shipping fee)
