What else do I need to know?

• • Never mail originals.  Your public library or local Staples have copiers, if required. 

  • 8" x 11" paper is preferred

  • Documentation for you and your spouse, if applicable, must be submitted together to avoid delays in processing.  

  • Incomplete submissions will be returned requesting the missing information. You must re-submit for reimbursement.  

  • Reimbursement checks are only made payable to the former employee even if only the spouse is Medicare eligible.

  • 2024 checks will be mailed starting in April 2025.

  • Mail all documentation to:
