What else do I need to know?

• • Never mail originals.  Your public library or local Staples have copiers, if required. 

  • Tri-folded, 8" x 11" copies in a legal-size envelope are preferred

  • Documentation for you and your spouse, if applicable, must be submitted together to avoid delays in processing.  

  • Incomplete submissions will be returned requesting the missing information. You must re-submit for reimbursement.  

  • Reimbursement checks are only made payable to the former employee, even if only the spouse is Medicare eligible.

  • 2025 checks will be mailed starting late April 2026.

  • If you would like confirmation that your mailed documents were received, please just make a note with your submission and include your email or phone number.  Otherwise, after reviewing your documents, you will only be contacted if we have any questions.
