Never mail originals. Your public library or local Staples have copiers, if required.
Tri-folded, 8" x 11" copies in a legal-size envelope are preferred
Documentation for you and your spouse, if applicable, must be submitted together to avoid delays in processing.
Incomplete submissions will be returned requesting the missing information. You must re-submit for reimbursement.
Reimbursement checks are only made payable to the former employee, even if only the spouse is Medicare eligible.
2025 checks will be mailed starting late April 2026.
